Paying with a Check or Money Order

Please Note, The following 3 items MUST received before ANY work can begin:

1. This completed form (below), signed and mailed.
2. Payment in full, for Artwork and Shipping.
3. All necessary Photos and Visual Reference (Sent Email, or Snail Mail)*
         *If sent via "Postal Mail", please send all 3 items under one cover.
(A delay in receiving any of the 3, will result in a later delivery date).

How to order a Caricature,
paying with Check or Money Order

1. First, go to the "Prices" page, (click: HERE)

2. At the "Prices" page, fill out the Inquiry form, and SEND.
       (Special instructions, requests, or shipping arrangements?  Include this information in the "#7. Specifics" box).   -OR-

2b. Call Caricatures by Lindsay at: 248-288-3571  to discuss your project in detail.

3. You'll receive an emailed response (and answers to any questions) with a
      price quote, and postage quote, within 2 business days.

4. Once you receive the quotes, PRINT this "ORDER FORM", below
      (in any of the ways suggested below).

5. Fill it out the "Order Form", completely (which you've downloaded from this page).
      Be sure to include the price quotes, along with the"order code".  Sign and Date Form.

6. Mail the Order Form to:   Caricatures by Lindsay   P.O. Box 175   Troy, MI 48099   USA

    NO work can BEGIN until ALL of following are received (preferably UNDER ONE COVER):
      A. ALL NECCESSARY PHOTOS AND REFERENCE MATERIALS
      B. THE COMPLETED AND SIGNED ORDER FORM
      C. CHECK OR MONEY ORDER FOR TOTAL AMOUNT.

4. I would like to receive a printable order form by email, and
      I have no problem accepting large (1 MB) files in my email.
      Send the order form in a large, non-compressed jpeg file.
      Please enter your eMail address in box #A and skip to #Eight.

      But I have major problems accepting large files in my email.
      I can however, accept "ZipIt" compressed files.
      Please choose from available compressed files and enter your
      eMail address in the box #B, and skip to #Eight"submit" below.

6. Mail me the order form via U.S. Postal Service (no charge)
      Please enter your name and mailing address in box #C
      and skip to #8 "submit" below.

7. FAX me the order form
     Please enter your FAX number, name, and check box for best time,
     and "submit" below.
